PDO Orientation Sign-in Sheet

Sign up sheets for PDO's can be as simple as a list of students and parents and their signatures. This document must be dated and also signed by the person responsible for the PDO. You can pull your list of students and export it to an excel spread sheet using directions in the wiki entitled: How to Pull a List of Participants to Invite to Pre-Departure Orientations NH 2010. Sign up sheets should be sent to Floyd Van Weelden, National Manager of Sending Gateways/ PDO Compliance, 3909 Reche Road, #114, Fallbrook, CA 92028 or emailed to Floyd at fvanweelden@afs.org. Please contact Floyd if you are having difficulty pulling your lists or if you need any other help with sign in sheets. Evaluations of the PDO by students, parents, and group leaders should also be sent to Floyd.
