Lead sheet

A lead sheet is a list of people interested in participating in or volunteering for AFS. It shows contact information for each person so that we can follow up with next steps.

If the lead sheet is being filled out at a place where computers are not easily available, then collecting names and contact information on a piece of paper is best. However, the sooner the information is transferred online, the better.

Rather than passing around a lead sheet, some people collecting names prefer to pass out contact cards.

Sending Lead Sheet: What to Collect

When collecting sending lead information, here is the information to collect. The information shown as "required" must be collected in order for the lead to be added to LMS.

  • First Name (required)
  • Last Name
  • Address1 (required)
  • Address2
  • City (required)
  • State (required)
  • Zip (required)
  • Home Phone (strongly encouraged)
  • Work Phone
  • Fax
  • Email (strongly encouraged)
  • Source Group (required: enter either "AFS Organized Events" or "AFS Staff/Volunteers"
  • Method Contacted (required): enter one of the following options
    • School Presentation Sign Up
    • Information Event Sign Up
    • AFS Booth
    • Other - See Comments
  • Volunteering Interest
  • Primary Interest (required)
  • Send Catalog (required): "yes" if Outbound Programs should send a catalog, or "no" if not
  • School Name
  • Graduation Date
  • School Host with AFS: enter one of the following options
    • Host with AFS
    • No
    • Don’t Know
    • Host with Other
  • Interested Year
  • Country Pref1
  • Country Pref2
  • Country Pref3
  • Comment (255 Char Max)
  • Contact Date
